In Best Medicine a brilliant surgeon leaves Boston to become a small-town doctor where he spent childhood summers. Despite his medical skills, his rude manner alienates locals as he battles hidden phobias and struggles with personal connections.
After Martin invites Louisa to watch the annual bird migration over Port Wenn, he treats an elderly woman whose hallucinations lead to dire consequences. Meanwhile, Aunt Sarah fumes after the reading of a will leaves her nothing and Martin must perform an emergency procedure on his young friend.
Port Wenn goes on lockdown when a patient with fleas sparks fears of a plague outbreak and Martin must deal with the town gossip when it revolves around him and Louisa. Also, Aunt Sarah feels betrayed by Greg and George and Louisa has an epiphany.
Martin is skeptical when Port Wenn welcomes an extremely friendly delegation from their sister city in Norway and Louisa visits a specialist who just happens to be Martin's ex. Also, Greg believes that he is pooping little men and Mark exhibits troubling symptoms that warrant an intervention.
As the furious people of Port Wenn discover the truth about their deal with their Norwegian sister city, Martin must toggle between the potential revelation of his blood phobia, a startling baby delivery and the arrival of his amorous ex. Also, Louisa receives some thought-provoking medical results and Al and Elaine must navigate their uncertain future.
While Martin continues to struggle with his blood phobia, a mermaid washes ashore in Port Wenn to the amazement of everyone in town. Meanwhile, Judith, a well-meaning townsperson creates havoc while on prescribed oxygen, Louisa continues to see Martin's girlfriend Eden for medical guidance and Greg and George's success from the seas urchin boom take its toll on the both of them.
When the Port Wenn pharmacy is robbed, including Louisa's critical time-sensitive medicine, Martin deduces that the culprit desperately needs his help. Meanwhile, Greg and George clash with a rival food truck owner, Sarah tries to bond with Eden, and Martin is on the receiving end of a special brand of Port Wenn affection.
