Becker centers on the life of Dr. John Becker, a cantankerous, yet dedicated and talented physician, who, in spite of his constant rants, has a decent heart underneath the sarcasm. Becker will continue to unleash his views at the local diner, even in Reggie's continuing absense. Jake, the blind proprietor of the local newsstand continues to turn a deaf ear to John. Bob, Becker's brash, obnoxious apartment super, persists in annoying virtually everyone. At the office, Becker's head nurse Margaret continues to make order out of chaos, keeping John and his space-cadet nurses-aid Linda in line. New to the neighborhood is Chris Konnors, Becker's new neighbor, with whom he must contend on a daily basis. Although her positive attitude is a major annoyance to Becker, he slowly begins to have feelings for her. John Becker is a cynical Harvard Medical School graduate who is somewhat of a loner and has trouble letting people get close to him. He looks at the world around him and feels society has gone mad - full of inconsistencies and just plain backward thinking. Although he is a diagnostician by trade, Becker feels he can dispense diagnoses even when no one asks. He has no qualms about saying what comes to mind, never sugarcoats his opinions and often seems to offend somebody nearby. However, his friends and colleagues recognize the heart beneath the hate and so accept his gruff demeanor for what it is.
John is ordered by a judge to see a therapist (Rhea Perlman) to help him work on his problem with authority. Also, Jake grows weary of his new girlfriend's annoying habit of describing every little thing she's doing. Kayla: Kali Rocha. Judge: Judyann Elder. Williams: Eugene Buica.
John is more irritable than usual when he tries to quit smoking; Reggie believes a friendly customer stole all of her good luck and left her with his bad fortune.
Everyone's shocked when Jake announces that he got married over the weekend to a woman (Lindsay Price) he met at a friend's wedding.
Reggie and Jake help a man (Taylor Nichols) who claims to be homeless, but John thinks he's a con artist; Margaret takes a second job caring for a wealthy woman's dog.
John takes Reggie's advice that he should tell women that he's sorry when he disappoints them, only to have it backfire; Jake and Bob switch jobs for the day. Beth: Mary Mara.
John hurts his shoulder and is afraid to have an MRI, so Reggie goes with him for support; Linda dates identical twins. Ricky: Randy Sklar. Donnie: Jason Sklar.
Jake encounters a depressed man (Dave Foley) who is about to hang himself, so he takes him back to the diner to cheer him up; John learns that a patient's wife is pregnant by another man. Jane Clayson appears as herself in a cameo.
Reggie becomes attracted to a married man (Robert Mailhouse) and considers having an affair; Linda starts bossing everyone around after she takes assertiveness classes.
John believes his no-strings, no-pressure relationship with his latest girlfriend (Lauren Holly) is perfect---until she leaves him for another man. At the diner, Bob boasts about finding a Super Bowl ring in the backseat of a cab. And at John's office, Linda tries to talk her parents out of getting a divorce.
John reluctantly agrees to buy a poor child a Christmas gift and then discovers that he has to deliver it as well; Bob visits his mother (Brenda Vaccaro) who still treats him like a kid.
John's day is filled with wacky people, from two guys who want a prescription for marijuana to a man who is upset when he learns that he isn't dying. Meanwhile, Jake challenges Bob to a game of scrabble.
Reggie starts dating one of John's recently divorced friends (James Read), but she can't stop obsessing about the guy's former wife. Sal: Robert Gallo.
John agrees to give free medical care to Bob, Reggie and Jake in exchange for repairs, food and magazines.
John tells a discerning bartender (George Wendt) about his horrible day and tries to get out of going to his ex-wife's wedding on Valentine's Day.
John worries that women no longer find him attractive after Jake's girlfriend (Lindsay Price) makes a disparaging remark to him; Linda fears that a pretty temp is going to replace her.
Parents are enraged when they learn that John's talk on nutrition to a class of 8-year-olds turns into a discussion about sex; two cops use the diner for a stakeout. Matlin: Bruce A. Young. When a patient (Tom Poston) loses the ability to speak, John gets him a machine that talks for him, but the man uses it to lash out at people; Bob turns into an environmentalist to impress a girl; Linda accuses Margaret of stealing her bracelet.
Reggie is obsessed with helping Jake achieve some of the goals he set for himself in sixth grade, but her good intentions backfire; Linda saves a man's life and then drives everyone crazy with her superior attitude.
John is a reluctant sex therapist for a couple in their 80s; Reggie is harassed by a man (Jon Polito) trying to sell her a new freezer.
John grows paranoid after his laundry, mail and apartment are tampered with, and the sunny disposition of a new woman (Nancy Travis) at the diner only makes him feel worse.
John receives eight expensive steaks from a patient and is compelled by his new neighbor into throwing a party.
Reggie gets jealous when she assumes John has feelings for a new neighbor and tries to tell him how she feels; Linda sublets her job to a woman even more incompetent than she is.
The fourth season concludes with a romantic cliffhanger as Reggie and a new neighbor each vie for John's affection. Also, Linda takes Margaret out for a night on the town, which Margaret lives to regret.
